How to email CEF and Consent Form to a Contact
After creating the contact via the Contact page, you will need to email the Client Establishment Form and Consent Form for them to complete before their first event.
From the Contacts page -
1. Select More Actions in the Command bar and then select the option from the list of available options, as follows:
2. Select Email CEF/Consent Form from the drop down list.
Note - Make sure the Contact has a valid email address prior to sending email.
3. A message will appear saying ‘Are you sure to send client establishment form/consent form to this contact’.
Click OK
4. Another message will appear to say the ‘Email Successfully sent to the client’.
Click OK
Once it has been sent, the email will be visible in the Communication tab of a Contact’s file.
The Contact will receive an email in their inbox; within the email they will need to click on the link to open the form.
For them to proceed further they will need to click on Send OTP to receive their One Time Password on their Mobile phone and click Submit.
This is what the Contact will see once they have entered their OTP.
The client will need to confirm consent (blue Confirm Consent button) before they are able to complete the fields in client establishment form.
Note: The Marketing Consent is optional if the client decides if they want to be contacted by Interrelate or their research partner to participate in a follow up evaluation.
If the client has previously consented to Interrelate, the consent section will not appear.
Once a new client confirms consent this will update the Consent Information box on their Summary Tab and the Marketing Box on the General Tab
After the client submits the Client Establishment Form the information will save to their Contact profile and a notification will appear in the Communication Tab
Note: A copy of the Client Establishment form is not saved to the Contacts file
